Blastomes de l’adulte

Présentation clinique et radiologique

Contrairement aux autres carcinomes sarcomatoïdes, les pneumoblastomes de l’adulte surviennent chez des sujets jeunes, entre 30 et 50 ans (13,21,28,29,47,48). Il existe une moindre prépondérance masculine, avec un sex-ratio à 2 :1 (1,49). La plupart des cas rapportés de pneumoblastome de l’adulte étaient observés chez des fumeurs (1,3).

Le blastome pulmonaire est une tumeur biphasique comportant un adénocarcinome de type fœtal (CK7+, TTF1+, NE souvent +) et une composante blastémateuse mésenchymateuse (vimentine+, actine +, AE1/AE3 focal). Les deux composants sont β-caténine + (cytoplasmique et nucléaire).

La découverte de la tumeur est souvent fortuite chez des patients asymptomatiques (40% des cas). Les patients symptomatiques sont porteurs de tumeurs volontiers volumineuses (allant jusqu’à 25 cm). La lésion est typiquement unique, bien délimitée, homogène, et périphérique. L’association à un épanchement pleural ou un envahissement pariétal n’est pas la règle. Le diagnostic différentiel avec un sarcome est parfois difficile, mais l’envahissement médiastinal est fréquent en cas de pneumoblastome, et exceptionnel en cas de sarcome.


Facteurs pronostiques

Dans la plus importante série publiée, qui incluait des cas non traités, la survie médiane était de 11 mois (50). La survie globale à 5 ans, tous stades confondus, se situe entre 21 et 54 % selon les séries (22,23). La survie globale ne varie pas en fonction de l’importance du contingent épithélial ou sarcomatoïde.

En revanche, elle dépend de la taille tumorale (seuil critique à 6-7 cm) et de l’extension ganglionnaire. La chirurgie avec résection complète est associée à une survie supérieure aux autres carcinomes sarcomatoïdes et aux carcinomes bronchiques non à petites cellules.


Stades précoces

Alors que la croissance tumorale est souvent rapide, la chirurgie est le traitement de choix en cas de tumeur de stade précoce, conduisant à une survie comprise entre 20 et 33 mois (50–62).

La résection est cependant souvent impossible ou incomplète en raison de la taille tumorale.  Peu de données disponibles sur les traitements adjuvants : quelques observations isolées décrivent l’utilisation de la radiothérapie ou de la chimiothérapie post-opératoire après résection incomplète ou en cas d’envahissement médiastinal N2 (52,53,59,60). Les protocoles de chimiothérapie utilisés associaient cyclophosphamide/ doxorubicine/cisplatine (57) ou cisplatine/étoposide (52) ou vindesine/cisplatine (61). La radiothérapie était délivrée à la dose de 60 Gy (53).


Stades métastatiques ou inopérables

Chez les patients inopérables, la radiothérapie exclusive a été rapportée dans quelques cas cliniques, avec une efficacité en termes de contrôle local (63). La chimiothérapie a également montré une efficacité en terme de contrôle local, avec des protocoles associant vincristine/dactinomycine/cyclophosphamide,  et plus récemment doxorubicine/ifosfamide/uromitexan (Mesna) ou cisplatine/étoposide, ou topotécan (53,58,64).

La chimio-radiothérapie séquentielle avec adriamycine/étoposide/cisplatine et irradiation à la dose de 60 Gy a été rapportée comme étant associée à une réponse tumorale de 75% (65).


Recommandations sur la prise en charge thérapeutique des carcinomes sarcomatoïdes

Les pneumoblastomes sont également des tumeurs agressives. Les modalités de la chimiothérapie sont spécifiques.
