Facteurs pronostiques

Les carcinomes sarcomatoïdes sont des tumeurs agressives, avec des métastases systémiques précoces, survenant non seulement aux sites métastatiques habituels des cancers bronchiques non à petites cellules (cerveau, os, foie), mais aussi dans des sites inhabituels comme l’œsophage, l’intestin grêle, le péritoine, gastriques, pancréatiques, gingivales, le tissu sous-cutané, le rein (1,4,17–19). Dans les séries publiées, la répartition des stades tumoraux au diagnostic apparaît identique à celle des autres cancers bronchiques non à petites cellules. Il faut cependant tenir compte du fait que les patients atteints de carcinome sarcomatoïde sont sous-diagnostiqués au stade métastatique du fait de la nécessité de tissu tumoral en quantité suffisante pour identifier les différentes composantes tissulaires de ces tumeurs (1,3,20–23).

L’évolution des carcinomes sarcomatoïdes est marquée par une croissance tumorale rapide (13,24,25), avec des récidives soit locales [15 % de l’ensemble des récidives (26,27)], soit plus fréquemment systémiques, survenant chez plus de 60 % des patients opérés (1,2,21). La médiane de la survie globale est comprise entre 6 et 20 mois, avec une survie à 5 ans inférieure à 10-20 % (2–4,13,18,20,21,28,29). Il faut là aussi tenir compte de la méconnaissance du diagnostic de carcinome sarcomatoïde au stade métastatique. Dans une analyse de la base de données SEER publiée en 2012, la survie des patients atteints de carcinome sarcomatoïde était inférieure à celle de patients appariés atteints de carcinomes bronchiques non sarcomatoïdes (30).

Les principaux facteurs pronostiques identifiés sont la taille tumorale, le stade, et l’envahissement ganglionnaire médiastinal. Dans une série récente de 70 cas de carcinomes pléïomorphes, le sous-type histologique épithélial prédominant (adénocarcinome, carcinome épidermoïde, ou carcinome à grandes cellules) n’était pas un facteur pronostique significatif (31).
